In order to open your RF file, you will have to download FrameMaker Document, Sun Raster Graphic, RealFlash Clip, or another similar software package.
If you have the relevant software, but your computer still won't open the software, you will have to change the file associations on Windows or Mac. Depending on your operating system, you can ensure that your computer always open RF files using FrameMaker Document, Sun Raster Graphic, RealFlash Clip, or another program of your choice that uses RF files.

In some instances, you may receive an error message stating that the file is corrupt. If you have tried all of the above, and it still doesn't work, download or request a new copy to see if that works.
If you don't want to download FrameMaker Document, Sun Raster Graphic, or RealFlash Clip (for whatever reason), and the other options also don't work... that's okay!
Even though some RF files must be opened in a program for which it was developed (binary format), you may still be able to open it in a universal file viewer such as File Magic. Download File Magic now from the Microsoft Store and open your RF file!

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.
Report an incident regarding a health care facility
The Incident Report Form is for health care facilities to notify the Indiana State Department of Health of a reportable incident pursuant to the ISDH Reportable Unusual Occurrence Policy. The Incident Report Form is also for health care facility staff (nursing homes, intermediate care facilities, and hospice agencies) to report a reasonable suspicion of a crime against a resident pursuant to Federal regulations. This form is not to be used to file a complaint.
